Clearances
Please make sure that all clearances are updated and copies are sent to the Human Resources Department prior to their expiration date(s). The Human Resources department does not receive any clearances directly from the agencies. Therefore, it is the responsibility of the staff member to provide their updated clearances to Human Resources.PLEASE BE ADVISED THAT SOME OF THE WEBSITES LISTED BELOW ARE HAVING TROUBLE WITH THE CHROME BROWSER. 1. Act 34 Criminal Background Check.
2. Act 151 Child Abuse History Clearance.

Local Earned Income Tax Residency Certification Form
Complete Employee Information Section. Find your PSD Code on the 2nd page of the form. Sign and date the Certification Section.
